Healing Like Wolverine? What Primary Care Clinicians Need to Know About BPC-157
If you haven't had a patient ask about BPC-157 yet, give it a little time. This "healing peptide," nicknamed after Wolverine's fictional regenerative powers, seems to promise everything from faster tendon and ligament repair to relief from digestive and neurodegenerative issues. It has quickly become one of the most talked-about peptides in wellness circles — and increasingly, in exam rooms.
What Is BPC-157, Exactly?
BPC-157 (short for "body protection compound-157") is a synthetic peptide derived from a naturally occurring gastric protein, one originally studied for its role in supporting mucosal integrity and gut homeostasis. It doesn't have FDA approval for any use, and it's banned in professional sports.¹
Its online presence really is staggering. More than 50 million videos tagged "BPC-157" across YouTube and TikTok combined, and peptide-focused Reddit communities have swelled past 100,000 members. Enthusiasts describe a compound that acts on more than ten different biological mediators,² including VEGF, growth hormone, and nitric oxide synthase, driving effects like angiogenesis, vasodilation, and reduced inflammation — the kind of tissue repair story that’s easy to get excited about.
It's a compelling mechanistic story. The problem is what happens when you go looking for the evidence to back it up.
The Evidence Gap
Here's where things get sobering. A 2025 systematic review³ in the HSS Journal compiled three decades of research, from 1993 to 2024, and identified only 36 studies. Of those, only one was a human trial, involving 7 to 12 patients who received intra-articular injections for knee pain, with some reporting symptom improvement. Beyond that, researchers have studied BPC-157 in humans in just two other small pilot studies: interstitial cystitis and intravenous safety and pharmacokinetics.
That's it. No study to date has systematically assessed adverse events or long-term safety in humans, and large-scale trials simply don't exist.
We do know a bit about how BPC-157 behaves pharmacologically: it's hepatically metabolized, has a half-life of under 30 minutes, is excreted in urine and has a detection window of up to 4 days. That's a meaningful detail for competitive athletes. Animal studies suggest its downstream regenerative effects may persist for weeks to months, even after the peptide itself has cleared the body.
Where Are Patients Actually Getting This?
Some cash-pay clinics now offer BPC-157 as intraarticular injections or oral formulations, marketed for knee pain, arthritis, and general musculoskeletal recovery. Since BPC-157 isn't DEA-scheduled, simply possessing it isn't illegal, unlike anabolic steroids, and it occupies a similar space in the athletic-enhancement conversation but carries very different legal consequences.
That legal gray zone, combined with unregulated manufacturing, means patients using BPC-157 may be exposed to contamination risks with essentially no clinical safety data to fall back on.
What Do You Say to Patients?
When a patient brings up BPC-157, counsel honestly about the risks of unregulated manufacturing and the thin safety data. For athletes specifically, flag the importance of understanding their organization's substance policies before they end up in a testing violation they didn't see coming.
The larger take-home is one worth sitting with: the evidence for BPC-157 in humans is genuinely limited, and the regulatory and safety questions are far from settled. That doesn't mean dismissing patient curiosity outright, but it does mean resisting the pull to co-sign the hype. More rigorous basic science and clinical research are needed before BPC-157 earns anything more than cautious, well-informed conversation in the primary care setting.
So next time a patient tells you about a peptide that "heals like Wolverine," you'll have more than a shrug to offer them.
